Powerhouse Performance Hockey
Online Registration Form

Please fill out the following form to sign up for any PowerHouse Performance Hockey program. You will be contacted to confirm the participation of your player in the program, and to arrange payment. We currently accept Visa, AMEX, Cheque or Money Order.

Payment and Cancellation Policies

Reservations:
Will be accepted by telephone and email, or spaces will be allocated on a first come first served basis. We do give priority to longer tenured students.

Payment:
Will be in full upon arrival to the class.

Cancellation (Winter Program):
Must be made at lease 24 hours before the session. Failure to notify us will result in charges for that particular session.

Cancellation (Camps):
All camps to be prepaid - No refunds, no future classes.
